I had thought it would have been a bullet stuck in the barrel after he had the squib round that he had to manually eject, but he fired several more shots after that.

I'm guessing seriously crap ammo.</description>

Obvious case rupture.  Perhaps due to case separation, bolt failure, or firing out of battery.  He's damned lucky.   That kind of thing has GOT to be a very rare event indeed.</description>
